Survey- ‘Transitions- A families view’
The Southwest region of Parent Carer Forums (which includes Parent Carers Together) and NHS England are working together to understand transitions for those families whose children have special educational needs or disabilities.
Please help us by completing this online survey and sharing your experience. You can access the survey at www.surveymonkey.co.uk/r/SWtransitions or use the QR code.
The survey will focus on the experience families have during a transition such as:
· moving to a new class/education setting
· changes in a service such as a new department or worker
· moving from children to adult services
There are 21 questions. The information you provide is anonymous.
The survey will close at midnight on Wednesday 21st December 2022.
